Instructions | Grade Request Form
This form is to be filled out by the student based on their high school transcript. If the course is an AP, or Honors course enter a 1 in the "AP or Honors" column, otherwise leave it blank. If the Grade received is equivalent to an "A" enter a 4 in the "Grade" column, for a "B" enter a 3, for a "C" enter a 2, for a "D" enter a 1, and for an "F" enter a zero. For freshman and sophomore year the typical entry for the column labeled "Portion of Credit" is "1". The exception is when the course is a half year, or half block course; in that case enter a "0.5". For your junior year the entry should reflect the portion of the course completed. [For example; for the first report card, in block scheduling enter "0.5", in "normal scheduling enter "0.25". For the second report card in block scheduling enter a "1" or in normal scheduling enter a "0.5". If the course being reported is a half credit course then the values entered would half the amount stated.] When the student has completed the form they should submit it to their guidance counselor for their review and confirmation. The form should be updated and resubmitted whenever the student has new information available to report.
